The assurance comes as the inquest into the death of the 49-year-old mother and grandmother resumed yesterday.

It found that she died as a result of ‘catastrophic’ internal bleeding caused by a laceration to a major blood vessel and perforated uterus during a minor gynaecological procedure on 17 October 2006.

The hearing also provided an opportunity for a number of questions to be asked of the Health Department to ensure that all recommendations highlighted by the independent Verita report, compiled as a result of Mrs Rourke’s death and made public last February, were being implemented.
