Hospital procedure change after death

Catherine Margaret Butler died in hospital following a cardiac arrest days after celebrating her 60th birthday, an inquest heard yesterday.

Following her death an investigation was carried out and a number of recommendations were made after it was revealed that doctors failed to pick up on vital test results which showed that Miss Butler was suffering from low potassium.

A pathologist found that Miss Butler died following a brain injury with associated cardiac arrest and low potassium levels in her blood.
